I freely acknowledge that I understand and accept the following:
• JoHannah Boots is not a medical doctor, licensed counselor; or medical practitioner and has not represented herself in that manner.
• JoHannah Boots does not diagnose conditions, or illnesses, prescribe medication, perform medical treatments, and will not interfere with a licensed medical professional.
• It is highly recommended that I see a licensed physician or licensed healthcare professional for any physical or psychological ailment I may have
• JoHannah Boots has made no promises to me as to the outcome of a Life Coaching Session
• My decision to receive a Life Coaching Session from JoHannah Boots is of my own free will.
I, for myself, my heirs, successors, executors, administrators, and assignee, hereby release and forever discharge JoHannah Boots, her heirs, successors, executors, administrators, and assigners from any and all allegations, causes of actions, claims, and demands for any reason of any damage, loss or injury, to person and property which heretofore has been or hereafter may be sustained in consequence of any treatment I may receive, in any respect of and for any attempts by myself or anyone on my behalf to cause temporary or permanent relief from the symptoms of any injury, ailment or disease with which I have been diagnosed.
